Today I visited the friendly periodontist. A nice little follow-up to the broken tooth incident. And the receptionist handed me a bunch of forms to fill out prior to the examination. The usual stuff about insurance and who to call if I keel over in the office. That kind of thing.
And then there was the Health Form. Checking off Yes or No to the typical questions about past diseases, etc. And then we got to the part that made me go...huh?!?! A series of questions...each phrased the same way....with multiple choice answers to circle. For example:
Have you ever knowingly been exposed to [insert various disease here].
And the choices for answers: YES NO DON'T KNOW
Wha???
If each question asks if I've KNOWINGLY been exposed...aren't the only possible answers either Yes or No? Is it even possible to not know whether or not I knowingly did something?!?!
Who creates these forms???

All I can say is "duh!" That is too funny. The funnier thing....I probably would not have picked that up!! Uhhhh...DUH again!! lol

You're a teacher, aren't you? You should have whipped out your red pen and corrected the form. Or maybe you did....
so how are your teeth?
If you knowingly know, and given that 2 positives make a negative, that would become an unknown, which must be about the same as knowingly unknown. I think. Oh, what was that? Oh, yeah, I had it backerds -- it's 2 negatives make a positive.
The unknown form designers are known to be, in the immortal words of jazz, idjits.
Thanks for asking, Meander. I have all of them in the right place...safe and sound. But I did visit a periodontist and I'm sure you'll hear all about his treatments once those appointments begin. But that won't be till February, so hopefully no tooth-related incidents in my life prior to that!
When ever those questionnaires ask sex, I always answer sometimes, how about you...
and to think that the dentist probaly does not even read the thing before he sticks is hand - that by the way have been exposed to God knows what- in our mouths! - Baron
i think there could be a dont know answer... for instance, one of my clients (im a caregiver to disabled people) contracted HepC. i do not think i was exposed to it, but i might have been. so i dont really know for sure, but i know i had contact with someone who was ill.
